Subject: Reads unmunged file from disk instead of dzil

This delta creates a serious regression:

In that, it is no longer able to see the source .pod file after Pod::Weaver munges it. And this radically changes how formatting works.

Or at least, that's the impression I get from how it now renders my pod in a garbage order in the README.

I'm also affected by this issue.
ReadmeFromPod is currently incompatible with Pod::Weaver.

Pod::Readme seems to have an input_fh parameter (inherited from Pod::Readme::Filter) that could be used.
